TATTOO CONSENT RELEASE FORM:
— I acknowledge by signing this release form that I have been given the full opportunity to ask all questions I might have about obtaining a tattoo from Scorpio Tattoo Studio.
— I acknowledge that all my questions have been answered to my full and total satisfaction
— I specifically acknowledge that I have been advised on the facts and matters set forth below, and I agree as follows:
— I am not under the influence of alcohol or drugs.
— I do not have acne, freckles, moles, or sunburn in the area to be tattooed that might be agitated by the tattoo process (healing excluded.)
— I have looked over my design, checked the spelling and if applicable, and given my full consent to the application of my tattoo.
— I acknowledge that I am not pregnant.
— I acknowledge that I am free of communicable disease.
— I acknowledge that I have truthfully represented to the associates, agents and representatives of Scorpio Tattoo Studio that I am over eighteen (18) years of age.
— I acknowledge it is not reasonably possible for the associate, agent, and representative of Scorpio Tattoo Studio to determine whether I might have an allergic reaction to the dyes, pigments, and or processes used in my tattoo and I agree to accept that such risks are possible.
— I acknowledge that infection is always possible as a result of obtaining a tattoo particularly in the event that I do not take proper care of my tattoo, and I have been advised of the signs and symptoms of the infection that indicate a need to seek medical care.
— I acknowledge receipt of instructions advising me of proper care of my tattoo and recognize the absolute necessity of following those written instructions. All questions about the body art procedure have been answered to my satisfaction.
— I acknowledge that variations in color and design may exist between any tattoos as selected by me and as ultimately applied to my body.
— I acknowledge that tattooing is a permanent change to my appearance and that no representations have been made to me as to the ability to later change, alter or remove my tattoo.
— I acknowledge that the obtaining of my tattoo is my choice alone and I consent to the application of the tattoo and to any actions or conduct of the associates, agents or representatives of Scorpio Tattoo Studio that are reasonable and necessary to perform the tattoo procedure.
— I agree to release and forever discharge and forever hold harmless Scorpio Tattoo Studio and its associates, agents, officers, and shareholders from any and all claims, damages, or legal actions arising from or connected in any way with my tattoo or the procedures and conduct used to apply my tattoo and any and all tattoos applied by Scorpio Tattoo Studio and its associates, agents and representatives in the future.
— I acknowledge that tattoo inks, dyes, and pigments have not been approved by the Federal Food and Drug Administration and the health consequences of using these products are unknown.
— I acknowledge that there is a chance I might feel lightheaded, dizzy during or after being tattooed. I agree to immediately notify the practitioner in the event I feel lightheaded, dizzy and/or faint before, during, or after the procedure.
— I do not knowingly have COVID-19. I am in good health and am exhibiting no symptoms of illness.
— I agree to follow all instructions concerning the care of my tattoo, and that any touch-ups needed because of my own negligence will be done at my own expense.
— I acknowledge that I have approved the design I am having tattooed and take full responsibility for mistakes in spelling and or mistakes in appearance.
Keep the bandage on for a minimum of 6 hours. We recommend keeping it on overnight provided the
dressing feels comfortable and secure.
Make sure your hands are clean. Use unscented antibacterial soap and avoid soap with fragrance,
moisture beads or scrubs.
1. Carefully remove the bandage and tape. If bandage sticks to the tattoo, wet it with lukewarm water
and gently slide it off. DO NOT force it off or be rough with it.
2. Make a lather in your hands with soap and warm water. No washcloths, bath towels, or sponges on
a fresh tattoo.
3. Gently clean the tattoo until all blood, ointment and plasma are removed. Rinse tattoo.
4. Pat dry with a clean towel or paper towel then allow to fully air dry. DO NOT SCRUB.
5. Once the tattoo is dry, apply a very small amount of aquaphor into your tattoo. Dab any excess off
with a clean paper towel.
CLEAN YOUR TATTOO THIS WAY ONCE OR TWICE A DAY IN THE SHOWER FOR THE FIRST 2-3 DAYS.
AFTERCARE!!!
Apply the aquaphor or unscented lotion 2-3 times a day (Remember, keep it light!) for the first 1-2 days
or until your tattoo starts to flake/peel like sunburn. When the peeling begins, switch to a regular
fragrance free lotion such as lubriderm, or any other fragrance free brand for an additional 2 weeks.
Your tattoo should be completely healed within 2-4 weeks.
Reduce the number of your daily lotion application if you notice your skin breaking out.
After the first few days, your tattoo may become itchy or begin to form flaky scabs. They will fall off on
their own, SO DO NOT PICK OR SCRATCH AT YOUR TATTOO, keep it moisturized and the scabs will
eventually dissipate.
NO SUN, NO SWIMMING, or SOAKING until the tattoo is completely healed.
Consult a physician at the first sign of infection.
